Job summary

Fortis Care in Georgia is seeking a flexible Nurse Practitioner or Physician Assistant to provide on-site clinical coverage at a skilled nursing facility. The role focuses on evaluating new admissions and acute issues, with physician support as needed, and involves independent practice during coverage hours.

Two options: Part-Time weekend coverage (Sat-Sun 9am-5pm) with no overnight calls, or a Full-Time track with potential to cover a second nearby building.

Qualifications

• Current Nurse Practitioner or Physician Assistant license in the state of Georgia.
• Board certification (AGNP or PA-C preferred).
• Unrestricted DEA registration.
• Minimum 1–2 years of clinical experience preferred.
• Experience in skilled nursing, post-acute care, geriatrics, internal medicine, hospital medicine, or emergency medicine strongly preferred.
• Familiarity with EHR systems and clinical documentation best practices.

Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during coverage hours.
• Respond to clinical issues identified by nursing staff and determine the appropriate evaluation, treatment, and follow-up plan.
• Manage common acute conditions in the skilled nursing setting, including infections, changes in mental status, respiratory symptoms, falls, pain, dehydration, and other changes in condition.
• Determine when residents can safely be treated in the facility and when escalation to the emergency department or hospital is appropriate.
• Review relevant laboratory results, imaging, medications, and recent clinical history to guide treatment decisions.
• Communicate clearly with nursing staff, physicians, residents, and families regarding changes in condition and treatment plans.
• Provide follow-up for residents recently discharged from the hospital or experiencing a significant change in condition when clinically appropriate.
• Maintain timely, accurate, and compliant documentation of all patient encounters.
• Collaborate with the facility and Fortis Care clinical team to ensure appropriate handoff and continuity of care.
