Job summary

Fortis Care in Georgia is seeking a part-time Advanced Practice Provider (NP or PA) to be the on-site clinician at a private-subacute rehab facility in the Northeast Atlanta area. The role offers a four-day work week (8:00 a.m.–6:00 p.m.), weekdays only, with 1–4 days per week available.

You will deliver direct medical care to short-stay rehab patients, coordinate care with the interdisciplinary team, and support transitions of care.

Qualifications

• Current Nurse Practitioner or Physician Assistant license in Georgia.
• NPs: certification in Internal Medicine, Geriatrics, or Family Practice; PAs: NCCPA certification.
• Unrestricted DEA license is required.
• Minimum 1–2 years of NP/PA experience; subacute rehab/geriatrics preferred.
• Ability to work independently with physician support.
• Familiarity with EHR systems and clinical documentation best practices.

Responsibilities

• Provide direct medical care to short-stay post-acute rehab patients (orthopedic, cardiac, pulmonary, stroke, neuro).
• Be the consistent clinical presence during the 8:00 a.m.–6:00 p.m. shift.
• Perform rounds, set stay expectations, and check in with patients and families.
• Manage acute and chronic conditions on-site to avoid unnecessary hospital transfers.
• Coordinate care plans with leadership, therapy, and hospital partners.
• Support transitions of care and medication reconciliation at discharge.
• Collaborate with nursing, therapy, social work, and concierge staff.
• Document encounters thoroughly and timely; participate in quality initiatives.
