Job summary

Fortis Care is seeking a part-time Weekend Nurse Practitioner to provide on-site clinical coverage at a skilled nursing facility. Coverage is Saturdays and Sundays from 9:00 AM to 2:00 PM, focusing on acute weekend issues to ensure timely treatment and reduce ED visits.

The role is highly independent with physician support as needed, offering a consistent weekend schedule without overnight calls and no weekday duties. Experience in geriatrics and SNF is preferred.

Qualifications

• GA NP license required.
• Board certification in Family NP or Adult-Gerontology NP.
• DEA registration required.
• Minimum 1–2 years NP experience preferred.
• Experience in skilled nursing, geriatrics, internal medicine, or emergency medicine is strongly preferred.
• Familiarity with EHR systems and charting best practices.

Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage hours.
• Respond to clinical issues identified by nursing staff and determine evaluation, treatment, and follow-up plan.
• Manage common acute conditions in the SNF setting, including infections, changes in mental status, respiratory symptoms, falls, pain, dehydration, and other changes in condition.
• Determine when residents can be treated in the facility and when escalation to ED/hospital is appropriate.
• Review relevant laboratory results, imaging, medications, and recent clinical history to guide treatment decisions.
• Communicate clearly with nursing staff, physicians, residents, and families regarding changes in condition and treatment plans.
• Provide follow-up for residents recently discharged from hospital or with significant condition changes when clinically appropriate.
• Maintain timely, accurate, and compliant documentation of all patient encounters.
• Collaborate with the facility and Fortis Care clinical team to ensure handoff and continuity of care.

What You’ll Be Doing

We are looking for a part-time Weekend Nurse Practitioner (NP) to provide on-site clinical coverage at a skilled nursing facility on Saturdays and Sundays from 9:00 AM–2:00 PM.

This role is focused primarily on evaluating and managing acute clinical issues that arise over the weekend, helping residents receive timely treatment in the facility whenever appropriate and avoiding unnecessary emergency department visits or hospital transfers.

This is a highly independent role with physician support available as needed. It is a great fit for an experienced clinician looking for a consistent, predictable weekend schedule without overnight call.

Key Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage hours.

• Commitment to high-quality, patient-centered care and reducing avoidable hospitalizations when clinically appropriate.

Qualifications

• Current Nurse Practitioner license in the state of Georgia.
• Board certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or other applicable NP certification.
• Unrestricted DEA registration.
• Minimum 1–2 years of Nurse Practitioner experience preferred.
• Experience in skilled nursing, post-acute care, geriatrics, internal medicine, hospital medicine, urgent care, or emergency medicine strongly preferred.
• Familiarity with EHR systems and clinical documentation best practices.
