Job summary

Fortiscares is seeking a part-time Weekend Nurse Practitioner to provide on-site clinical coverage at a skilled nursing facility on Saturdays and Sundays from 9:00 AM to 5:00 PM. The role focuses on evaluating and managing new admissions and weekend acute issues, with physician support as needed and no overnight call.

This independent, weekend schedule offers consistency for experienced clinicians and opportunities to coordinate care with nursing staff and families to reduce unnecessary hospital

Qualifications

• Current Nurse Practitioner license in Georgia.
• Board certification as a Family Nurse Practitioner or Adult-Gerontology NP, or other applicable NP certification.
• Unrestricted DEA registration.
• Minimum 1–2 years NP experience preferred.
• Experience in skilled nursing, geriatrics or internal medicine preferred.
• Familiarity with EHR systems and clinical documentation best practices.

Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage.
• Respond to clinical issues identified by nursing staff and determine evaluation and follow-up plan.
• Manage common acute conditions in skilled nursing settings and determine escalation needs.
• Review labs, imaging, medications, and clinical history to guide treatment decisions.
• Communicate with nursing staff, physicians, residents, and families about changes in condition and plan.
