  • Communicate clearly with nursing staff, physicians, residents, and families regarding changes in condition and treatment plans.
  • Provide follow-up for residents recently discharged from the hospital or experiencing a significant change in condition when clinically appropriate.
  • Maintain timely, accurate, and compliant documentation of all patient encounters.
  • Collaborate with the facility and Fortis Care clinical team to ensure appropriate handoff and continuity of care.
Skills & Abilities

  • Ability to quickly assess changes in condition and determine the appropriate level of care.
  • Excellent communication and collaboration skills with nursing staff, patients, families, and physicians.
  • Strong organizational and documentation skills.
  • Comfortable working independently in a skilled nursing facility with physician support available as needed.
  • Commitment to high-quality, patient-centered care and reducing avoidable hospitalizations when clinically appropriate.

  • Board certification (AGNP or PA-C preferred).
  • Unrestricted DEA registration.
  • Minimum 1–2 years of clinical experience preferred.
  • Experience in skilled nursing, post-acute care, geriatrics, internal medicine, hospital medicine, or emergency medicine strongly preferred.
