Job summary

Jefferson Health Plans in Turnersville, NJ is seeking a Per Diem/Day Shift Case Coordinator to support coordination of post-hospital care. You will work under the Manager of Case Management and Case Manager RN to arrange homecare, DME, transportation, infusion services, and authorizations for identified patients.

The position requires a High School Diploma (Bachelor's degree preferred), hospital or patient-facing experience, and comfort with medical terminology and insurance portals.

Qualifications

• High School Diploma required; Bachelor's degree preferred.
• Experience in hospital settings or patient-facing roles preferred.
• Experience obtaining post-acute insurance authorizations via insurance portals.
• Must be computer literate with strong knowledge of medical terminology.

Responsibilities

• Coordinate homecare, DME, transportation, infusion services, and other post-acute services for identified patients.
• Interact with patients and families to deliver and obtain patient rights documentation.
• Interact with co-workers and visitors in alignment with Jefferson values.
• Must be able to multi-task and prioritize in a fast-paced environment.
