Qualifications

• Bachelor’s degree in Nursing is required.
• Minimum 3 years case management or discharge planning experience.
• Demonstrates conflict resolution and teamwork in care planning.
• Understands developmental cues in illness/trauma across ages.
• Able to identify elder signs of abuse or neglect and safety issues.

Responsibilities

• Coordinate discharge planning and establish safe discharge plans.
• Advocate for patient and family throughout the care episode.
• Facilitate communication among the healthcare team and patient/family.
• Monitor progression and address barriers to discharge.
• Lead or support Patient Progression Leader during rounds.
• Escalate unresolved barriers to Department Leadership.
