Jefferson Health’s Abington-Jefferson campus in Montgomery County, PA is seeking a Part Time RN Case Manager (Days) to coordinate care for a defined patient population and manage discharge planning across the inpatient stay.

The role requires a Bachelor’s Degree of Nursing and at least 3 years of case management or discharge planning experience, with strong communication and interdisciplinary collaboration skills. Work hours are 8:00am–4:30pm, 24 hours weekly.

Qualifications

• Bachelor’s Degree of Nursing is required.
• 3 years case management or discharge planning experience.
• Demonstrates effective conflict resolution skills as necessary.
• Uses interdisciplinary collaborative team approach to patient care planning and discharge coordination.
• RN - Licensed Registered Nurse - PA license is required.

Responsibilities

• Coordinates all aspects of discharge planning and establishes a safe discharge plan for assigned patients.
• Advocates for the patient and family throughout the entire episode of care.
• Functions as a resource to facilitate communication among the healthcare team and patient/family.
• Facilitates patient progression throughout the patient’s hospital stay.
• Monitors patient’s progress daily and addresses barriers to discharge to ensure timely progression.
