Job summary

Jefferson Health is seeking a Case Manager RN for per diem positions across New Jersey to coordinate discharge planning and patient progression. You will apply clinical expertise to develop safe discharge plans and facilitate communication among the care team and patients.

Responsibilities include coordinating post-acute services, documenting criteria, and mentoring new staff while ensuring timely and appropriate patient progression through the hospital stay.

Qualifications

• Bachelor’s Degree of Nursing is required.
• 3 years case management or discharge planning experience.
• Demonstrates conflict resolution, prioritization and communication skills.

Responsibilities

• Interacts with co-workers, patients and family to advocate for patient care.
• Coordinates discharge planning and ensures safe post-acute arrangements.
• Documents medical necessity criteria and manages referrals and authorizations.
