Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for PRN weekend staffing at our facility. Primary need is weekend staffing, but other availabilities will be considered.

The PT will assess referred patients, develop and administer treatment plans under physician direction, document progress, educate patients and families, and supervise Physical Therapy Assistants. Collaboration with the care team is essential.

Qualifications

• Assess referred patients to determine rehabilitation needs.
• Set realistic, achievable goals and administer treatment under physician direction.
• Document patient condition and educate patients and families.
• Supervise Physical Therapy Assistants.
• Ensure confidentiality and HIPAA compliance.

Responsibilities

• Assess patients and provide direct patient care for rehabilitation.
• Develop and monitor treatment plans and goals.
• Document progress, discharge planning and recertification as needed.
• Collaborate with interdisciplinary team to provide patient-centered care.
