Job summary

The Gallatin Center for Rehabilitation and Healing seeks a Physical Therapist (PT) PRN to support weekend staffing at our facility. The role emphasizes direct patient care, evaluations, and customized treatment plans for residents in long-term care.

The PT will supervise PT Assistants, document progress, and work with a multidisciplinary team to deliver patient-centered care while upholding confidentiality and professional standards.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold valid state license and registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric care preferred.
• Strong documentation writing and record-keeping.
• Ability to conduct evaluations and weekly progress notes.
• Collaborates with all disciplines to provide patient-centered care.
• Punctual, professional and HIPAA compliant.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment plans with goals based on capacity.
• Document patient condition, educate patients and families.
• Oversee and supervise Physical Therapy Assistants.
• Perform duties assigned by Director of Rehabilitation.
• Maintain confidentiality and residents' rights.
