We are currently recruiting for a compassionate Weekend Home Health Physical Therapist (PT) to reliably serve patients in Philadelphia, PA. This is a pay-per-point position.*

What you'll do:

As a Jefferson Health at Home by BAYADA Physical Therapist, you'll use your clinical skills and compassionate heart to treat patients where they want to be the most - in the comfort of their own home. Responsibilities include:

1. Seeing a client through their care needs - from hospital discharge to living a safe home life with comfort, independence, and dignity.
2. Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, and safety assessments.
3. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assessing and revising the PT care plan regularly, and collaborating with nursing in the multidisciplinary care plan as appropriate.
5. Implementing integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.

What you should have:

• Active and current license to practice as a Physical Therapist (PT) in PA.
• Experience with OASIS is strongly preferred.
• Ability to travel to assigned cases.
