Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a licensed Physical Therapist (PT) for PRN weekend staffing. Responsibilities include patient assessment, direct care, and developing individualized rehabilitation plans within physician direction.

You will document patient progress, educate patients and families, and collaborate with all disciplines to deliver best patient-centered care in a long-term care setting.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation and record-keeping skills.
• Collaborates with all disciplines to provide patient-centered care.
• Adheres to confidentiality and HIPAA.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document patient condition and progress; update weekly notes and discharge docs.
• Educate patients and families on physical therapy methods.
• Supervise Physical Therapy Assistants and coordinate with the care team.
• Maintain professionalism, punctuality, and compliance with schedules.
