Job summary

Three Oaks Hospice, Inc. is seeking an On-Call Registered Nurse (RN) for weekend coverage to join our team of hospice professionals.

The RN will administer skilled nursing care to patients in their homes and coordinate with the interdisciplinary team, patient, family, and referring agencies. Qualified candidates hold an RN license, CPR certification, and two years of RN experience in a clinical setting; hospice/home health experience is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer; Microsoft Office experience is preferred
• Strong interpersonal skills and ability to provide effective support to patient and family
• Demonstrate skill in nursing practice and coping with family emotional stress

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume responsibility for coordination of care.
