Azura Memory Care is seeking a compassionate RN to provide on-call hospice coverage every other weekend in the Madison/Verona area. You will respond to patient needs with phone support and in-person visits.

Current Wisconsin RN license in good standing required; mileage reimbursement, and opportunities for additional hours available. Join a supportive team and make meaningful connections with patients and families.

Qualifications

• Current Wisconsin RN license in good standing.
• On-call availability every other weekend, Friday through Sunday.
• Hospice, palliative care, home health, or strong medical-surgical experience preferred.

Responsibilities

• Respond to patient and family needs during your on-call weekend, including phone support and in-person visits
• Provide symptom management, admissions, and death visits as needed
• Deliver skilled nursing care in patients' homes, assisted living, and memory care communities
• Educate and support families while coordinating care with our team and physicians
