Generations Hospice is seeking an On-Call Hospice RN to join our team in Wisconsin. You will provide 1:1 end-of-life care during after-hours, collaborate with a remote triage team, and support families with compassion and expertise.

We offer a generous sign-on bonus, flexible scheduling, and a supportive interdisciplinary team. A valid RN license and driving ability are required, with travel across the territory expected.

Competenze

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred
• Experience in hospice or similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured and in good working order

Mansioni

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medications or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and next steps
• Pronounce death, provide family support, notify the hospice team if death occurs during on-call hours
• Complete timely documentation and communication
