Job summary

Trilogy Health Services, LLC is seeking a Weekend Supervisor (LPN/RN) to support the DON and ADON in overseeing weekend nursing functions. The role focuses on maintaining high-quality care, staffing coordination, and ensuring compliance with care plans across the Health Campus in Peru, IN.

Ideal candidates hold a current LPN or RN license with CPR certification and are available for 12-hour weekend shifts. Compensation includes an incentive program and benefits aligned with our Thrive platform.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Current, valid CPR certification required.
• Experience in a long-term care setting is preferred.

Responsibilities

• Assist the ADON & DON to plan, develop, organize, implement, evaluate, and direct the nursing service department.
• Supervise, oversee and assist with resident care according to plan of care.
• Provide direct nursing care as necessary and assess treatment outcomes.
• Assist in scanning and attaching resident documentation in MatrixCare for accurate medical records.
• Maintain first impressions and support survey readiness and campus preparedness.
• Ensure campus is tour ready and execute tours as needed.
• Provide patient care as necessary.
