Job summary

Fortiscares is seeking a Part-time Weekend Nurse Practitioner to provide on-site coverage at a Georgia skilled nursing facility on Saturdays and Sundays from 9:00 AM to 2:00 PM.

The role focuses on evaluating and managing acute weekend issues, delivering timely care, and preventing unnecessary ED visits with physician support as needed. The position offers a consistent weekend schedule, no overnight calls, and independence with collaboration.

Qualifications

• Current NP license in Georgia is required.
• Board certification as FNP/AGNP or equivalent.
• Unrestricted DEA registration is required.
• 1–2 years NP experience preferred, especially in skilled nursing or geriatrics.
• Experience with EHR systems and clinical documentation.

Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage hours.
• Respond to clinical issues identified by nursing staff and determine plan of care.
• Manage common acute conditions in skilled nursing settings and decide on ED/hospital escalation.
• Review labs, imaging and meds to guide treatment decisions.
• Communicate with staff, physicians, residents and families about changes in condition.
• Provide follow-up for residents recently discharged or with significant condition changes.
• Maintain accurate documentation of all patient encounters.
• Collaborate with Fortis Care clinical team to ensure continuity of care.
