Job summary

Fortis-Care is seeking a part-time Weekend Nurse Practitioner to provide on-site clinical coverage at a skilled nursing facility on Saturdays and Sundays from approximately 9:00 AM to 5:00 PM. The role focuses on evaluating and managing new admissions and weekend acute issues to ensure timely treatment and avoid unnecessary ED visits.

Qualifications

• Georgia NP license required, board-certified NP preferred.
• Unrestricted DEA registration is required.
• 1–2 years NP experience preferred in relevant settings.
• Experience in SNF, geriatrics, internal medicine, or acute care is strongly preferred.
• Familiarity with EHR and clinical documentation best practices.

Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage hours.
• Respond to nursing staff concerns and determine evaluation, treatment, and follow-up plans.
• Manage common acute SNF conditions and decide on in-facility care vs escalation.
• Review labs, imaging, meds, and recent history to guide treatment decisions.
• Communicate changes and plans clearly with staff, residents, and families.
• Provide follow-up for residents recently discharged from hospital or with significant changes.
• Maintain timely, accurate, and compliant documentation of encounters.
• Collaborate with facility and Fortis Care team for handoffs and continuity of care.
