Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality care to residents in a skilled nursing facility. The NP will assess and manage acute and chronic conditions, collaborating with an interdisciplinary team to improve outcomes and quality of life.

The role emphasizes weekend, part-time scheduling with a competitive compensation package and opportunities for additional income through performance-based incentives.

Qualifications

• Master’s degree in Nursing from an accredited program.
• Active and unrestricted APN license in the respective state.
• Current DEA registration and prescriptive authority (or eligibility).
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
• 2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of chronic disease management and geriatric care principles.
• Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizations.
• Proficiency in evidence-based practice and clinical decision-making.
• Ability to work both independently and collaboratively within an interdisciplinary team.
• Excellent communication and interpersonal skills with residents, families, and staff.
• Strong organizational, time management, and documentation skills.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Perform comprehensive health assessments, diagnose and manage conditions in a SNF setting.
• Order and interpret diagnostics, prescribe treatments, conduct medication reviews.
• Deliver timely clinical interventions to improve outcomes.
• Support value-based care initiatives and quality improvement.
• Collaborate with physicians and team members for coordinated care.
• Assist in care plan meetings and transitions across settings.
• Educate residents and families on diagnoses, treatments, and prevention.
• Maintain accurate documentation in compliance with standards.
