Job summary

Vitae Health is seeking a dedicated Nurse Practitioner to deliver high-quality medical care to residents in skilled nursing facilities. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary team to improve outcomes and quality of life.

Qualifications

• Must hold an RN/NP licensure with APN authority in the state.
• Board-certified NP with active, unrestricted license.
• DEA registration and prescriptive authority or eligibility.
• Experience in geriatrics/long-term care preferred.
• Strong clinical assessment, diagnosis, and treatment planning.

Responsibilities

• Perform comprehensive health assessments and develop care plans for SNF residents.
• Provide medical management including orders, reviews, and monitoring to reduce hospitalizations.
• Deliver timely clinical interventions to changes in patient status.
• Participate in care plan meetings and quality improvement efforts.
• Educate residents and families on diagnoses and preventive care.
