Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our healthcare team providing high-quality primary care in skilled nursing and senior living facilities. This weekend-only, part-time role focuses on wellness, chronic disease management, and coordinated care with other providers to improve residents' lives.

Responsibilities include comprehensive assessments, routine rounds, and education for residents and families.

Qualifications

• Valid NP or PA license in the state of operations and current board certification.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality and organizational skills; ability to function within a team.

Responsibilities

• Perform comprehensive assessments and develop treatment plans for residents.
• Conduct routine rounds and adjust treatment plans as needed.
• Collaborate with facility staff, family, and other providers for coordinated care.
• Educate residents and families on disease prevention and wellness.
• Maintain EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care.
