Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other healthcare providers to enhance wellness and quality of life for seniors.

The role requires ongoing rounds, patient education, and thorough EMR documentation, with travel between assigned facilities.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Current board certification in your field.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills; punctual and organized.

Responsibilities

• Provide direct patient care and focus on wellness, chronic disease management.
• Perform comprehensive assessments and develop treatment plans.
• Conduct routine rounds and monitor residents' conditions.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and families on disease prevention and wellness.
• Document accurately in EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives to reduce readmissions.
