Job summary

Qualifications
- Nurse practitioner or physician assistant license in state of operation and board certification.
- Master's degree in nursing or PA studies.
- Strong leadership, interpersonal, and problem-solving skills.
- Excellent written and oral communication skills; punctual and organized.
- Ability to work collaboratively within a care team.
Responsibilities
- Perform comprehensive assessments and develop treatment plans for residents.
- Conduct routine rounds, adjust care plans, and monitor conditions.
- Coordinate care with facility staff, families, and other providers.
- Educate residents and families on disease prevention and management.
- Maintain EMR documentation and ensure regulatory compliance.
- Participate in quality improvement to enhance care and reduce readmissions.
