Job summary

Curana Health is seeking an experienced Nurse Practitioner or Physician Assistant to provide weekend rounding at a long-term care facility. You will assess 8–10 residents each Saturday and Sunday, manage acute changes, and coordinate with on-site staff to ensure continuity of care for an established patient panel.

No weekday commitment required. The role emphasizes preventive care and chronic disease management, with a focus on high-quality, patient-centered outcomes.

Qualifications

• Strong leadership, interpersonal, and problem-solving skills are preferred.
• Excellent written and oral communication skills are essential.
• Master's degree required for NP or PA licensure in relevant field.
• Current NP or PA license in the operating state and board certification are required.

Responsibilities

• Provide weekend rounding coverage for residents at a long-term care/skilled nursing facility.
• Conduct comprehensive assessments and manage acute changes in condition.
• Coordinate with facility nursing staff to ensure continuity of care for a stable patient panel.
• Educate residents and families about disease prevention and wellness strategies.
• Maintain EMR documentation accurately and in compliance with standards.
• Participate in quality improvement initiatives to enhance care delivery and outcomes.
