Qualifications
- Masters degree required in nursing or physician assistant studies.
- Licensed NP or PA with current board certification in the state of operation.
- Strong leadership, interpersonal and problem-solving skills preferred.
Responsibilities
- Perform comprehensive resident assessments and develop care plans.
- Conduct routine rounds and monitor chronic conditions in SNF and senior living facilities.
- Coordinate with facility staff, families, and other providers to ensure integrated care.
- Educate residents and families on wellness and disease management.
- Document thoroughly in EMR and ensure regulatory compliance.
- Participate in quality improvement initiatives to enhance care quality.
