Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant for weekend rounding in our senior care network. You will provide primary care in skilled nursing and senior living facilities, manage chronic diseases, and promote wellness through coordinated care with interdisciplinary teams.

This role emphasizes patient-centered outcomes, documentation accuracy, and collaborative practice.

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations.
• Current board certification in the respective field.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal and problem-solving skills preferred.
• Excellent written and oral communication skills.

Responsibilities

• Perform comprehensive assessments and develop treatment plans for residents.
• Conduct routine rounds, monitor conditions and adjust plans as needed.
• Coordinate care with facility staff, families and other providers.
• Educate residents and families on disease prevention and wellness.
• Maintain accurate EMR documentation and regulatory compliance.
• Participate in quality improvement to enhance care and satisfaction.
