Infinite Medical P.C. is hiring a Primary Care NP/PA to provide on-site care to residents in long-term care facilities in Greenfield, MA on weekends. This part-time role emphasizes meaningful patient relationships within a collaborative care environment.

Candidates must hold an unrestricted MA license (NP/PA/Physician) with at least 6 months of LTC experience. Responsibilities include exams, chronic/acute condition management, lab interpretation, and coordinating with facility staff.

Qualifications

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• State-specific license in good standing in Massachusetts.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Strong communication, clinical, and critical thinking skills.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
