Infinite Medical P.C. is seeking a Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to provide on-site care in long-term care settings in Newburyport, MA.

This part-time weekend position offers collaborative workflows within a multidisciplinary team and a mission-driven environment focused on elderly care. The role requires active NP/PA license(s) and a MA state license in good standing, with a minimum of 6 months' experience in long-term care.

Qualifications

• Active and unrestricted NP/PA license(s) and MA license in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
