MedElite is seeking a Primary Care NP/PA to provide on-site care in long-term care facilities in Stoughton, MA. This part-time weekend role offers meaningful patient relationships and collaborative care within a supportive team.

Ideal candidates are licensed MA NPs or PAs with LTC experience, strong communication, and a passion for elder care. The position emphasizes patient-centered care and close coordination with facility staff.

Qualifications

• Active and unrestricted Nurse Practitioner or Physician Assistant license in MA.
• State-specific MA license in good standing.
• Minimum 6 months experience in long-term care or skilled nursing facilities.
• Strong communication, clinical and critical thinking skills.
• Passion for elder care and person-centered healthcare.
• Ability to stand/walk for extended periods; light lifting possibly up to 20 lbs.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Ensure a safe, respectful, and supportive environment.
