Infinite Medical P.C. is hiring a Primary Care NP/PA to provide on-site care for residents in long-term care facilities in Medford, MA. This part-time weekend role emphasizes meaningful patient relationships and collaborative care in a supportive team environment.

The ideal candidate holds an active NP/PA license, a MA license in good standing, and at least 6 months of long-term care experience, with strong communication and clinical skills.

Qualifications

• Active and unrestricted NP, PA, or physician license.
• State MA license in good standing.
• Minimum 6 months of experience in long-term care.
• Strong communication and critical thinking skills.
• Ability to stand/walk for extended periods and assist with patient handling.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
