The role supports service line implementations, EMR and OnShift usage, staff development, and continuous quality improvement to ensure high-quality resident care.

Qualifikationen

• Current RN licensure with the State of Delaware.
• Graduated from accredited nurse program.
• Current CPR card.
• Minimum one year Long Term Care experience.
• Clinical coordination experience for Medicare/Short Term Rehabilitation units.

Aufgaben

• Demonstrates leadership and problem solving with commitment to excellence of Encore at Foulk.
• Implements Service Lines (Pulmonary Rehab, Cardiac Care) to improve quality and reduce readmissions.
• Oversees EMR, Risk Watch and OnShift.
• Interviews, hires, orients and evaluates Unit staff to maintain resident-centered care.
• Ensures regulatory compliance with local/state/federal guidelines.
• Conducts Care Plan meetings with residents and families; oversees discharge planning.
