Job summary

Encore at Foulk is seeking an experienced RN Supervisor for night shifts on weekends. The role focuses on coordinating care in the skilled nursing unit, mentoring staff, and ensuring adherence to policies to provide resident-centered care.

Responsibilities include leading staff development, implementing service lines to enhance quality, and maintaining EMR accuracy. Knowledge of regulatory guidelines and strong communication with families and providers is essential.

Qualifications

• Current RN licensure in Delaware.
• Minimum one year Long Term Care experience.
• Clinical coordination experience for residents in Medicare/Short Term Rehabilitation.
• Experience with residents who have Alzheimer's/Memory Impairment.

Responsibilities

• Demonstrates leadership and problem solving to drive excellence in Encore at Foulk.
• Leads staff development and supervises unit personnel to maintain resident-centered care.
• Manages implementation of service lines (Pulmonary Rehab, Cardiac Care) to improve quality and reduce readmissions.
• Ensures EMR, Risk Watch, and OnShift usage and compliance.
• Conducts Care Plan meetings with family and residents; oversees discharge planning.
