Job summary
Eden Nursing & Rehabilitation in Brackenridge, Pennsylvania is seeking a Baylor Licensed Practical Nurse (LPN) to provide compassionate, evidence-based care to our residents under supervision of the healthcare team.
You will assess needs, develop care plans, administer medications, mentor Care Assistants, and collaborate with the interdisciplinary team to ensure optimal outcomes. Weekend full-time shift, 7am-11pm, with a competitive pay rate of $28-$32 per hour.
Qualifications
- Must be a licensed Practical Nurse in Pennsylvania.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a team.
Responsibilities
- Provide direct patient care and assess, plan, implement, and evaluate care plans.
- Develop and maintain accurate patient records.
- Collaborate with the interdisciplinary team to implement individualized care plans.
- Administer medications and treatments as ordered by physicians.
- Monitor vital signs and patient conditions.
- Provide patient education and support to patients and families.
- Participate in quality improvement initiatives to improve outcomes and satisfaction.
