Job summary

Rosewood Rehabilitation & Nursing Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our Weekend Baylor Program team. As an LPN, you will provide high-quality patient care to residents under supervision from our healthcare team and enjoy a unique weekend schedule.

Our Weekend Baylor Program offers 12-hour shifts on Saturdays and Sundays with increased hourly pay for weekend work, and flexible scheduling options.

Qualifications

• Current LPN licensure in Pennsylvania.
• Experience in healthcare; long-term care or rehabilitation setting a plus.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct patient care, including administering medications.
• Monitor and report changes in patient condition to the RN or physician.
• Collaborate with interdisciplinary team to develop and implement individualized care plans.
• Maintain accurate and detailed records of patient care and treatments.
• Participate in quality improvement initiatives to optimize patient outcomes and satisfaction.
• Foster a positive and respectful environment that promotes patient-centered care.
