Job summary

Rosewood Rehabilitation & Nursing Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our Weekend Baylor Program. You will provide direct patient care, administer medications, and monitor patients under supervision, supporting a dedicated healthcare team in Schuylkill Haven, Pennsylvania.

The role offers weekend Baylor schedule with 12-hour shifts (7pm-7am and 7am-7pm) and a pathway to enhanced weekend pay. Prior long-term care experience and PA LPN licensure are preferred.

Qualifications

• Current LPN licensure in Pennsylvania.
• Prior experience in healthcare settings; long-term care a plus.
• Strong communication and interpersonal skills with patients, families, and team.

Responsibilities

• Provide direct patient care, including administering medications.
• Monitor and report changes in patient condition to RN or physician.
• Collaborate with interdisciplinary team to develop/implement care plans.
• Maintain accurate records of patient care and treatments.
• Participate in quality improvement initiatives to optimize outcomes and satisfaction.
• Foster a positive environment that promotes patient-centered care.
