Job summary

Rosewood Rehabilitation & Nursing Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our Weekend Baylor Program. You will provide direct patient care, administer medications, and monitor patients under supervision, supporting a dedicated healthcare team in Schuylkill Haven, Pennsylvania.

The role offers weekend Baylor schedule with 12-hour shifts (7pm-7am and 7am-7pm) and a pathway to enhanced weekend pay. Prior long-term care experience and PA LPN licensure are preferred.

Qualifications

Current LPN licensure in Pennsylvania.
Prior experience in healthcare settings; long-term care a plus.
Strong communication and interpersonal skills with patients, families, and team.

Responsibilities

Provide direct patient care, including administering medications.
Monitor and report changes in patient condition to RN or physician.
Collaborate with interdisciplinary team to develop/implement care plans.
Maintain accurate records of patient care and treatments.
Participate in quality improvement initiatives to optimize outcomes and satisfaction.
Foster a positive environment that promotes patient-centered care.
