Job summary

Qualifications

• Current, active LPN license in the state of practice.
• Must be at least 21 years old.
• CPR/First Aid/AED certification.
• Familiarity with electronic medical records.

Responsibilities

• Administer medications and provide patient education.
• Monitor and document vitals and patient progress.
• Coordinate treatment and discharge planning with the multidisciplinary team.
• Maintain a safe and de-escalated environment for patients.
• Adhere to licensure requirements and facility policies.
