Job summary

Reliant Care Management Company is seeking a Weekend LPN to care for our residents. The shift runs 6:00 PM to 6:00 AM on Friday, Saturday, and Sunday. You will conduct assessments, document findings, and follow protocols while coordinating with the care team to ensure high-quality resident care.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
