Job summary

Reliant Care Management Company in Missouri is seeking a licensed practical nurse (LPN) to support residents’ needs with empathy and patience. The role includes assessments, admissions, discharges, wound care, and supervising CNAs and CMTs in accordance with facility protocols.

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
