Reliant Care Management Company seeks a caring LPN to support residents and the care team. You will perform assessments, follow protocols, manage admissions/discharges, and lead CNAs and CMTs while ensuring facility policies are followed.

Responsabilidades

• Conduct assessments, follow protocol, report changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and med orders.
• Manage discharges: complete documentation, notify physician and guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
