Reliant Care Management Company seeks a weekend LPN to join our care team in Missouri. The role covers in-house resident assessments, wound care, admissions, discharges, and supervising CNAs/CMTs, with a focus on compassionate, patient-centered care.

Shifts are Fri-Sun, 6:00 pm–6:00 am (and Fri-Sun 6:00 am–6:00 pm). A $10,000 sign-on bonus is offered to eligible candidates. We provide comprehensive benefits and supportive work environment.

Qualifications

• Must hold LPN licensure and be in good standing.
• Weekend availability required (Fri-Sun).
• Care-minded, empathetic and patient approach.
• Ability to lead CNAs and CMTs.

Responsabilités

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
