AuthoraCare Collective in Greensboro, NC, seeks a part-time Weekend Social Worker for Home Care & Long-Term Care to join our hospice team. The role requires a Bachelor’s or Master’s degree in social work and at least two years of relevant experience, with CPR certification and travel to patient homes within the service area.

The position offers weekend shifts (every other Saturday and Sunday 8:00am - 7:00pm) and a comprehensive benefits package including PTO, holidays, flexible schedules,

Qualifications

• Bachelor's or Master's degree in social work is required.
• Two years of experience in social work in a mental health or medical setting is required.
• CPR certification from AHA or Red Cross BLS is required.

Responsibilities

• Provide intervention and assistance in generalist social work practice, including assessment, planning, intervention, evaluation, case management, information/referral, counseling, advocacy and coordination of resources.
• Assist with long-term care planning, LTC placement, necessary documentation, and PASRRs.
• Travel to patients' homes within the service area to deliver care and support.
