Job summary

AuthoraCare Collective in Greensboro, NC, seeks a Hospice RN Home Care Case Manager to oversee care plans for a caseload of hospice patients. You will collaborate with the IDG, assess needs, set nursing goals, and coordinate medical plans with physicians, while maintaining strong patient and caregiver relationships.

Qualifications

• Graduate of an accredited School of Nursing.
• Must be currently licensed as an RN in North Carolina and in good standing with NCBON.
• Experience in an acute, hospice, home health setting, or long-term care setting a plus.
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Responsibilities

• Manage the overall plan of care for a caseload of hospice patients in collaboration with the interdisciplinary group (IDG).
• Assess and identify patient/caregiver problems, establish nursing goals and interventions.
• Collaborate with physicians to establish the medical plan of care for the patient.
• Establish and maintain therapeutic relationships with patient and caregiver, including emotional support and teaching.
