Job summary

AuthoraCare Collective in High Point, NC, is seeking a part-time Home Health Social Worker to provide psychosocial support to patients and families in a home health setting.

You will assess needs, develop individualized care plans, and connect clients with community resources, while collaborating with an interdisciplinary team to promote independence at home.

Qualifications

• One year of social work experience in a mental health or medical setting is required.
• Must be able to travel to homes within the service area.
• Must have valid state-issued driver's license and own transportation.

Responsibilities

• Conduct psychosocial assessments and counseling, assist with advance care planning, and connect patients to community resources.
• Collaborate with the interdisciplinary team and home health director.
• Develop and carry out individualized plans of care related to social determinants of health.
• Identify resources to meet patient/caregiver/family needs.
• Advocate for patients and families.
