AuthoraCare Collective is currently seeking a Weekend (Home Care & Long-term Care) Social Worker for our Greensboro service areas with flexibility to support our Burlington service areas as needed . This is a part-time position; every other Saturday and Sunday 8:00am - 7:00pm.
Job Summary:
Responsible for providing intervention and assistance to the Social Work Department which is consistent with basic generalist social work practice. This includes: resource assessment and planning, intervention, evaluation, case management, information and referral, supportive presence/supportive counseling, case management, advocacy, community resource and client. Also included is long-term care planning to consist of LTC placement, completion of appropriate documentation i.e. FL-2s and obtaining PASRRs. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
