Job summary

AuthoraCare Collective is seeking a Hospice RN Home Care Case Manager to join our Greensboro service area. This full-time position (32 hours per week / 4 days) focuses on managing the overall plan of care for hospice patients in collaboration with the interdisciplinary group, with emphasis on assessment, goal setting, and coordinated medical planning.

The RN must be licensed in North Carolina, CPR/BLS certified, possess a valid driver's license, and be prepared for on-call responsibilities.

Qualifications

• Graduate of an accredited School of Nursing.
• Must be licensed as an RN in North Carolina and in good standing with NCBON.
• Experience in an acute, hospice, home health, or long-term care setting is a plus.
• CPR/BLS certification required.

Responsibilities

• Manage the overall plan of care for hospice patients in collaboration with the interdisciplinary group (IDG).
• Assess and identify patient/caregiver problems, establish nursing goals and interventions.
• Work with physician to establish the medical plan of care for the patient.
• Establish and maintain a therapeutic relationship with patient and caregiver; provide emotional support and education.
• Document outcomes of care; communicate with team; provide dietary counseling as needed; assume on-call responsibilities.
