Job summary

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate Registered Nurse to provide rapid-response hospice field visits throughout Palm Beach County. You will assess, triage, and manage urgent health conditions in home and community settings with autonomy and collaboration.

The role emphasizes educating families, coordinating care with physicians and the interdisciplinary team, and delivering high-quality, dignified Full Life Care.

Qualifications

• Active Florida RN license.
• 1 year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of Hospice Nursing: case management, crisis intervention, teaching, and thanatology concepts.

Responsibilities

• Provide rapid-response nursing care, triage, and management of urgent health conditions in home/community settings.
• Collaborate with physicians, APPs, and interdisciplinary teams to deliver timely, high-quality care and reduce ED visits or hospitalizations.
• Coordinate comprehensive care under the direction of the primary physician in collaboration with the team.
• Monitor patient conditions, administer interventions, and deliver education to improve comfort and quality of life.
