Job summary

Empath Health's Trustbridge Hospice in West Palm Beach, FL is seeking a compassionate Hospice RN Case Manager to join our Palm Beach County team. In this role you will coordinate patient-centered hospice care under the direction of the primary physician with an interdisciplinary group, typically Monday through Friday 8:30am-5:00pm with weekend coverage.

The ideal candidate is a Florida-licensed RN with BLS, understands hospice nursing, and brings strong case management, teaching, and crisis

Qualifications

Registered Nurse licensed in Florida.
Current Basic Life Support Certification.
One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
Understanding of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills and thanatology.

Responsibilities

Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed.
Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group).
Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.
Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust.
