Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Registered Nurse to provide rapid hospice field visits across Palm Beach County. You will work 7pm–7:30am with every other weekend, delivering patient-centered care in home and community settings.

As a Field-Based Emergent Care RN, you will triage urgent conditions, collaborate with physicians and the care team, educate families, and help prevent unnecessary hospital visits while supporting dignity and comfort at the end of life.

Qualifications

Current Florida RN license.
Basic Life Support (BLS) certification.
At least 1 year in public health, home nursing, oncology, medical/surgical, geriatrics or hospice nursing preferred.
Experience in hospice or crisis intervention is a plus.

Responsibilities

Provide rapid-response, patient-centered clinical care in home and community settings.
Assess, triage, and manage urgent health conditions outside traditional clinics.
Collaborate with physicians and interdisciplinary teams to prevent ED visits.
Advocate for patients and families and coordinate care under physician direction.
Monitor patient conditions and provide education to improve comfort and quality of life.
